UX Testing and Feedback:Form

 

So I have made a UX testing form for my website in order to further improve it. 

The form has four sections,

Introduction

Hardware Assessment 

Quantitative Data

And finally Qualitative Data

Each section has questions based on it's heading and the overall theme of the form is similar to the website's overall theme.
